​Here you will find information on the formalities necessary to legalize your exchange abroad.

Please be advised that:

  • Erasmus + KA103 includes mobilities to Program Countries, i.e. European countries; grants are awarded to students qualified in the Erasmus+, CEMS MIM and double degree recruitment programs;

  • Erasmus + KA107 program includes mobilities to Partner Countries, i.e. outside Europe. In the case of these mobilities, the students will be informed individually about obtaining the grant.

​Please, submit the following documents to the International Centre:

1. Contact form – please, pick up the one relevant for your exchange program

2. Exchange reguest – it has to be filled out electronically and printed double-sided in 2 copies 

3. Learning Agreement – it has to be filled out electronically and printed double-sided in 2 copies

    • Erasmus+ KA103 [Programme Countries: Europe; template] – student needs to deliver the original

    • Erasmus+ KA107 [Partner Countries: overseas; template] – student needs to deliver the original

6. Mandatory health insurance (EHIC for Erasmus+) and accident insurance (e.g. ISIC or Euro26) – at latest 1 month before departure (copy delivered or sent by email to contact person). In case of Erasmus+ lack of such confirmation of insurance will result in delay in the scholarship payment.

We suggest students to obtain additional insurance covering the loss of stolen property; in case of going to "exotic" places students should get acquainted with information on mandatory vaccinations.

Confirmation about being selected for exchange abroad which is necessary to obtain EHIC and/or visa will be issued on student's request after he/she delivers final versions of all required documents.

 

 Dean's offices – pre-departure procedures

 
​First, submit 2 copies of the Learning Agreement and 2 copies of Exchange request to your contact person at the International Office.  When accepted and signed, pick them up and submit to:
  • Ms. Katarzyna Smolińska [Erasmus+, PIM, bilateral agreements, CEMS] and Ms. Magdalena Wilczyńska [Double Degree Programmes] at the Dean's Office of the Undergraduate Studies (room 5, build. G),

  • Ms. Urszula Szczecińska [Erasmus+, PIM, bilateral agreements, CEMS] and Ms. Monika Rosińska [Double Degree Programmes] at the Graduate Studies Dean's Office (room 56, build. G)

When accepted and signed by the Dean, pick them up and make photocopies of the documents. Next, submit a photocopy of the Learning Agreement and the orignal of the Exchange request to the International Centre.
